    Obtained new source material [see OP above for new link] - still a scan but significantly better than original posted copy. But this does still suffer from "improper" scanning of gutter; unable to lay book pages flat. (Looks like this was borrowed and scanned from Denver Library in Colorado). But outside purchasing this for 11,000 Yen, the images are Avedon at some of his best . . .
